Animals on Campus Request Form

Yes, you need to fill out this form if you intend to bring animals on campus for:

• • Public events, educational demonstrations, or temporary visits (e.g., therapy animals in the library, wellness events, etc.);
  • Indoor administrative or academic spaces not generally open to animals and not part of an accommodation; or
  • Unusual cases not clearly covered by other policies.

No, you do not need to fill out this form if you are:

• Requesting an accessibility accommodation. Follow one of these links to those separate procedures.
  • Student accessibility accommodation request via Disability Services
  • Employee accessibility accommodation request through Human Resources
• Bringing animals on campus for approved research. 
  • Animals involved in research, teaching, or at University-affiliated agricultural facilities are governed by other policies (e.g., Use of Live Vertebrate Animals Policy, Use of Animals at Off Campus Locations Policy, and IBC Policies for Animals in Research and BSL-2 Teaching Laboratories).
